Several million French people do not have a declared primary care physician with the Health Insurance. In areas where healthcare availability is dwindling, locating an available healthcare professional and contacting them can sometimes feel like an obstacle course. Official directories exist, as do online appointment platforms, but their practical use raises questions that the homepages of these services do not always address.

Direct access to certain healthcare professionals: what recent decrees change

Before even searching for a doctor, it is important to know that the care pathway has evolved. A decree that came into effect in January 2025 has expanded direct access to an advanced practice nurse (IPA). In certain coordinated practice settings or facilities, a patient can consult an IPA without prior protocol with a doctor.

This possibility remains limited to the practice setting. It is not enough to find an IPA in a directory to benefit from direct access: the structure in which they practice must meet specific criteria. Verifying these conditions directly with the office or health center in question remains the only reliable method.

The same logic applies to physiotherapists. According to professional information published by the FFMKR, coverage by Health Insurance depends on the place of practice (health center, coordinated care team) and distinguishes between situations with or without prior medical diagnosis. Again, asking the office whether direct access and reimbursement apply to your particular case avoids unpleasant surprises.

These changes do not yet concern all professions or all territories. They open doors, but each door has its own entry conditions. To guide your search towards professionals suited to your situation, you can obtain access to the Aux Portes de la Santé website, which lists different categories of practitioners.

Official Health Directory and private platforms: data that do not always overlap

The Health Directory of the Digital Health Agency (annuaire.esante.gouv.fr) and the Health Insurance directory (annuairesante.ameli.fr) are the two public reference databases. Santé.fr aggregates this information by adding filters by specialty, location, and type of structure.

Private platforms like Doctolib or Maiia offer online appointment scheduling and teleconsultation. Their coverage is broad, but it reflects the practitioners who have chosen to register there. A professional absent from Doctolib may appear in the official directory, and conversely, a practitioner listed on a private platform may no longer accept new patients.

What directories do not say

Finding an available professional in a directory does not guarantee that they accept new patients. Public databases do not always display this information. Private platforms reflect it better, as appointment slots are visible there, but a practitioner may block their online availability while still accepting patients by phone.

Cross-referencing multiple sources remains the most comprehensive method:

  • Official directory (esante.gouv.fr or ameli.fr) to verify the registration, specialty, and conventional sector of the practitioner
  • Appointment platforms (Doctolib, Maiia) to identify available slots and practitioners who accept new patients
  • Direct phone call to the office, the only way to confirm the acceptance of new patients when the information is not available online

Territorial professional health communities (CPTS): an unknown local relay

CPTS bring together healthcare professionals from the same territory. Some of them offer active support to people without a primary care physician, beyond just a simple list of practitioners.

The service and access criteria vary from one CPTS to another. Not all offer this type of assistance, and those that do do not apply the same rules. The approach is to identify the CPTS in your geographical area and then contact them directly to see if they can guide you.

This option is not prominently highlighted by online directories. It requires an active approach from the patient, but it can unlock situations where traditional searches do not yield results.

Teleconsultation and My Health Space: useful complements, not replacement solutions

Teleconsultation has become established in the landscape, offered by both appointment platforms and dedicated services. It allows for quick medical advice, but it does not replace follow-up by a primary care physician for chronic conditions or physical examinations.

My Health Space, the digital health record launched by Health Insurance, centralizes medical documents and facilitates information sharing among professionals. Its service catalog also references health applications. The tool becomes truly useful when a coordinated care pathway already exists.

What is missing for truly effective searching

No single tool currently allows you to know simultaneously if a professional is available, if they accept new patients, what their actual waiting time is, and if they charge extra fees. Public directories cover the regulatory aspect (sector, specialty). Private platforms cover appointment scheduling. CPTS cover local support.

  • For a general practitioner: start with the Ameli directory, filter by conventional sector, then check availability on an appointment platform
  • For a specialist: use the official Health Directory to identify practitioners in the area, then attempt to schedule an appointment online
  • For a paramedical professional (IPA, physiotherapist): first check if direct access applies to your situation before making an appointment
  • In case of deadlock: contact the local CPTS or the Health Insurance conciliator

The search for a healthcare professional in France still requires combining multiple tools and accepting that each only covers part of the necessary information. Recent regulatory developments on direct access to certain professionals open up alternatives, provided that you verify on a case-by-case basis whether they apply to your territory and situation.
